Essential Functions:

• Ensure that all dietary procedures are followed.
• Serve palatable and appetizing meals in a timely manner.
• Assist in daily or scheduled cleaning duties as established.
• Obtain food supplies for next meal.
• Maintain an area of safety and sanitation by correct use and storage of attire, tools, and equipment.

Position Requirements:

• High school diploma or equivalent preferred.
• Previous food service experience preferred.

Qualifications:

• Ability to read, write and verbally communicate with others.
• Willingness to learn and adhere to food safety and sanitation principles, proper operation of institutional equipment, and food preparation procedures.
• Responsible to work cooperatively with other team members, promoting teamwork.

Schedule:

• Weekend Clinical team members must work every Saturday and Sunday 6-month commitment.
• Premium pay – receive time and a half on the weekends.
• Will receive a total of four weekends or (8 weekend shifts) off per year

About the Role: As an MDS Coordinator, you’ll play a key role in creating and maintaining care plans for our residents, ensuring that their needs are met with compassion and precision. Your work will involve:

• Completing MDS and care plans in accordance with Federal guidelines.
• Using care plan software to enhance our care planning process.
• Collaborating with staff nurses to ensure timely completion of nursing summaries.
• Monitoring residents for significant changes and following up on clinical issues.

What We’re Looking For:

• A licensed Registered Nurse or LPN in Indiana with previous nursing and Long-Term Care experience.
• Someone with a strong understanding of MDS and care planning.
• A team player who is dedicated to improving residents' lives.

• Hourly rate based on experience
• $1.50 additional per hour for second and third shift.
• Weekend shift pay: $1.50 additional per hour
• Weekends and Holidays: Must work a rotating schedule
