Job Description:

Dietary Aides will be responsible for sanitary and safe handling of food and equipment and keeping area clean. Responsible to complete duties on a timely basis to meet meal time schedules without assistance.

Job Duties:

  • Work with dietitian as necessary and implement recommend changes as required.
  • Ensure that all dietary procedures are followed.
  • Develop and maintain rapport with coworkers to assure that food service will meet resident needs.
  • Serve palatable and appetizing meals in a timely manner.
  • Serve food in accordance with established portion control procedures.
  • Assist in daily or scheduled cleaning duties as established.
  • Clean work tables, meat blocks, and refrigerators/freezers.
  • Sweep and mop floors.
  • Carry dirty and used utensils, etc. to wash areas.
  • Perform dishwashing/cleaning procedures. Clean and wash utensils as directed.
  • Take out garbage and keep work area clean, dry, and free of hazards.
  • Set up meal trays, food carts, dining room, etc. as instructed.
  • Assist cooks in preparing meals.
  • Distribute and collect menus as necessary.
  • Obtain food supplies for next meal.
  • Assist in checking diet trays before distribution.
  • Deliver food carts, trays, etc., to designated areas. Remove and return to dishwashing area as needed.
  • Serve food in dining room as instructed.
  • Prepare and deliver snack as instructed.
  • Maintain an area of safety and sanitation by correct use and storage of attire, tools, and equipment.
  • Report all hazards to supervisor immediately.

Job Requirements/Qualifications:

  • High School diploma or equivalent preferred
  • Ability to read, write, and communicate effectively
  • Requires the ability to lift up to 35 pounds occasionally and up to 20 pounds frequently.
  • Occasional reaching, kneeling, standing, pulling, lifting, grasping, feeling required.

Schedule:

  • Dietary Assistants are required to work every other weekend.

Duties Include:

  • Maintain good resident hygiene by giving partial baths, bed baths, showers, tub baths, shampooing and styling hair, shaving, and doing oral care and nail care.
  • Feed or assist residents in eating by means of spoon feeding or with verbal cues and encouragement.
  • Repositioning and exercising residents by means of turning, providing range of motion exercises, and assisting with ambulation according to the as needed and tolerated..
  • Answering call lights and addressing residents' need and wants in a timely manner.
  • Perform and satisfactorily complete all daily assignments in a timely manner.
  • Maintain all assigned residents in good positioning and in clean clothing; groom residents after snacks, meals, and as needed throughout the tour of duty.
  • Keep supervisor informed of the resident's status and of any changes in the resident's condition.
  • Chart accurate and pertinent information on all required forms and flow sheets as instructed.
  • Maintain resident rooms in a clean and tidy manner to provide a sanitary environment by changing soiled bed linens, making a neat bed, straightening up closets and bedside stands, and storing personal oral care items to protect from contamination.
  • Transport residents to activities and special recreational events and support all other services through an interdisciplinary, holistic approach.
  • Remain on the unit and leave only for scheduled breaks and lunch with permission of supervisor.
  • Participate in all meetings, conferences, and in-services as required per federal regulations (12 hours per year) and to meet the needs of the residents.

Job Requirements/Qualifications:

  • Successful completion of a state-approved Basic Nurse Aide Class.
  • Certification as a Certified Nursing Assistant recognized by the state of Indiana upon hire OR within 120 days of hire.
  • Must maintain certification as required by the facility or the state.
  • Requires the ability to lift up to 80 pounds occasionally and 60 pounds frequently

  • Hourly rate based on experience
  • $0.50 additional per hour for second shift.

Job Description:

RN’s/LPN’s provide consistent, high-quality care by identifying resident needs and progress, medical assessments and promotion of team approach to resident care. Coordination and supervision of total nursing activities and personnel in designated unit including support to CNAs with resident care.

 

Duties Include:

  • Make regular rounds of assigned work unit to ascertain the condition of all residents.
  • Report all pertinent resident issues to the charge nurse responsible for residents in the designated unit on the next tour of duty.
  • Supervise the care of residents in designated unit; visit each resident to evaluate immediate physical, psychosocial, and nutritional condition; listen to resident comments relating to needs and problems and implement required nursing intervention.
  • Review resident health records and other resident-identifiable data bases, i.e., MDS, to determine appropriate interventions necessary.
  • Care for selected residents requiring his/her special skills and experience.
  • Notify the physician of resident status changes via telephone or facsimile machine; assist the physician with clinical visitation rounds.
  • Inform the resident, family member(s), or guardian of changes in the resident's condition.
  • Accurately transcribe and implement physicians' orders; prepare and pass medications; complete treatments as ordered.
  • Complete required documentation in an accurate, legible, and timely manner in accordance with professional standards of nursing practice and federal state regulation.
  • Contact pharmacies to obtain prescriptions and refills in accordance with physicians' orders.
  • Restock medication and treatment carts; maintain carts in a sanitary condition.
  • Count and check all narcotics at the beginning and end of each shift.
  • Execute the destruction of outdated and discontinued medications per facility policy.
  • Assist with transport of residents to dining areas, and with feeding and care of residents.
  • Review designated CNA and QMA charting and completion of assigned duties.
  • Inform DON of significant resident status changes, pertinent nursing personnel matters, and other related matters through written reports and/or verbal communication.
  • Delegate responsibility to nursing personnel through work assignments for direct nursing care of specific residents and non-nursing duties during his/her tour of duty.
  • Authorize meal break, rest breaks, or changes in scheduled working hours for same.
  • Assist in assessing the level and number of personnel assigned to each nursing unit; make recommendations regarding such personnel for better resident care and utilization of same.
  • Thoroughly investigate and document all incidents/ accidents and all medications errors.
  • Keep other departments informed of special needs of residents; coordinate the care and services with other disciplines.
  • Continuously observe the environment for conditions of cleanliness, sanitation, safety, and overall appearance, and report any adverse findings to the appropriate staff member for further action.
  • Participate in in-service education and staff orientation.
  • Attend professional meetings and seminars as dictated by facility needs and prepare a summary of such programs.
  • Participate in facility committees, i.e. interdisciplinary care plan review team, Infection Control, Quality Assurance, as requested by DON.

Job Requirements:

  • Graduation from an accredited school of professional nursing required.
  • Currently licensed as an RN in the state of Indiana with the Health Professions Bureau required.
  • Must maintain license as required by the facility or the state.
  • Long term care nursing experience preferred.
  • Requires the ability to lift up to 80 pounds occasionally and 60 pounds frequently.

This position has occupational exposure to blood borne pathogens and other potentially infectious materials. Adherence to universal precautions is required.

  • Full Time, Evening Shift
  • Must work every other weekend and every other holiday

Job Description:

RN’s/LPN’s provide consistent, high-quality care by identifying resident needs and progress, medical assessments and promotion of team approach to resident care. Coordination and supervision of total nursing activities and personnel in designated unit including support to CNAs with resident care.

Duties Include:

  • Make regular rounds of assigned work unit to ascertain the condition of all residents.
  • Report all pertinent resident issues to the charge nurse responsible for residents in the designated unit on the next tour of duty.
  • Supervise the care of residents in designated unit; visit each resident to evaluate immediate physical, psychosocial, and nutritional condition; listen to resident comments relating to needs and problems, and implement required nursing intervention.
  • Review resident health records and other resident-identifiable data bases, i.e., MDS, to determine appropriate interventions necessary.
  • Care for selected residents requiring his/her special skills and experience.
  • Notify the physician of resident status changes via telephone or facsimile machine; assist the physician with clinical visitation rounds.
  • Inform the resident, family member(s), or guardian of changes in the resident's condition.
  • Accurately transcribe and implement physicians' orders; prepare and pass medications; complete treatments as ordered.
  • Complete required documentation in an accurate, legible, and timely manner in accordance with professional standards of nursing practice and federal state regulation.
  • Contact pharmacies to obtain prescriptions and refills in accordance with physicians' orders.
  • Restock medication and treatment carts; maintain carts in a sanitary condition.
  • Count and check all narcotics at the beginning and end of each shift.
  • Execute the destruction of outdated and discontinued medications per facility policy.
  • Assist with transport of residents to dining areas, and with feeding and care of residents.
  • Review designated CNA and QMA charting and completion of assigned duties.
  • Inform DON of significant resident status changes, pertinent nursing personnel matters, and other related matters through written reports and/or verbal communication.
  • Delegate responsibility to nursing personnel through work assignments for direct nursing care of specific residents and non-nursing duties during his/her tour of duty.
  • Authorize meal break, rest breaks, or changes in scheduled working hours for same.
  • Assist in assessing the level and number of personnel assigned to each nursing unit; make recommendations regarding such personnel for better resident care and utilization of same.
  • Thoroughly investigate and document all incidents/ accidents and all medications errors.
  • Keep other departments informed of special needs of residents; coordinate the care and services with other disciplines.
  • Continuously observe the environment for conditions of cleanliness, sanitation, safety, and overall appearance, and report any adverse findings to the appropriate staff member for further action.
  • Participate in in-service education and staff orientation.
  • Attend professional meetings and seminars as dictated by facility needs and prepare a summary of such programs.
  • Participate in facility committees, i.e. interdisciplinary care plan review team, Infection Control, Quality Assurance, as requested by DON.

Job Requirements:

  • Graduation from an accredited school of professional nursing required.
  • Currently licensed as an RN in the state of Indiana with the Health Professions Bureau required.
  • Must maintain license as required by the facility or the state.
  • Long term care nursing experience preferred.
  • Requires the ability to lift up to 80 pounds occasionally and 60 pounds frequently.

This position has occupational exposure to blood borne pathogens and other potentially infectious materials. Adherence to universal precautions is required.

Job Description:

The nurse manager is responsible for assisting the nursing services in planning, organizing, developing and directing the day-to-day activities of the Oak Grove campus. These functions will be carried out in accordance with current federal and state regulations, professional nursing standards, and operational policies of Oak Grove.

Duties Include:

  • Assist the Director of Nursing Services in planning, developing, implementing, evaluating and directing the day-to-day functions of the Nursing Services Department and more specifically your assigned community, in accordance with current, rules, regulations, and guidelines that govern the facility.
  • Participate in developing, maintaining and updating written policies and procedures that govern the day-to-day functions of the Nursing Services Department.
  • Assist in developing methods for coordinating nursing services with other residents’ total regimen of care.
  • Ensure that all nursing services personnel assigned to the community follow their respective job descriptions.
  • Perform administrative duties such as completing medical forms, reports, employee evaluations, charting, etc. as necessary.
  • Review all incidents/accidents on your Community to ensure appropriate documentation, investigation and follow-up per policy.
  • Participate in the development, maintenance and implementation of the facility’s quality assurance program for the Nursing Services Department.
  • Participate in surveys (inspections) made by authorized governmental agencies.
  • Serve on, participate in and attend various committees of the facility, i.e. risk management, quality assurance as directed.
  • Evaluate and implement recommendations from established committees as they may pertain to nursing services.
  • Make daily rounds of the Nursing Services Department to ensure that all nursing service staff are performing their work assignments properly. Report findings to the Director of Nursing Services.
  • Provide the Director of Nursing Services with information relative to the nursing needs of the resident and the Nursing Services Department’s ability to meet those needs.
  • Inform nursing services personnel on your community of new admissions, their expected time of arrival, room assignments, plans of care, etc.
  • Periodically conduct audits to ensure compliance with established charting guidelines and state/federal regulations..
  • Make daily rounds/monitor direct care to observe residents and to determine if nursing needs are being met.
  • Routinely monitor medication passes and treatments to ensure compliance and proper technique.
  • Provide direct nursing care as necessary.
  • Monitor nursing staff to assure that they are following established safety regulations and infection control guidelines.
  • Ensure that all nursing staff follow established department policies and procedures, including 
  • appropriate dress codes.
  • Perform and document resident assessments as assigned and according to established policies and procedures.
  • Ensure that all staff involved in providing care to the resident is aware of the care plan and that care plans are used in administering daily care to the resident.
  • Review nurses notes to determine if the care plan is being followed.
  • Assure that nursing staff follow universal precautions, personal protective equipment is available and used when necessary.
  • Assist with onboarding of new hires & maintain records for TB/CPR/HEP B and orientation
  • Assist with in services as required by ISDH monthly and as needed
  • Assist with MDS assessments/care planning/pre-certifications/quarterly assessments.
  • Maybe required to cover a shift as a floor nurse in staffing challenged situations.

Job Requirements/Qualifications:

  • LPN or RN Indiana license in good standing
  • Training in geriatric nursing preferred
  • 3-5 years on the job nursing experience in long term care
  • Flexibility in schedule
  • Ability to work with a wide variety of personalities in a pleasant and professional manner
  • Professional image to enhance positive public relations
  • Must possess excellent communication skills written and verbal
  • Ability to manage time well and be organized
  • Ability to work independently and on a team

  • Pass ice water to residents, as needed.
  • Participate in one-on-one or small group engagements with residents.
  • Handling of soiled linens from laundry hampers, dining rooms, resident rooms and housekeeping staff, if necessary. Sort when appropriate.
  • Launder items when appropriate.
  • Fold personal and linen items and return to respective residents' room or areas using clean, covered cart when appropriate.
  • Perform cleaning functions in assigned areas following established schedules and using prescribed methods.
  • Interact with residents, staff and guests in a respectful adult manner. 
  • Work cooperatively with other staff and departments in coordinating and conducting unit-based activities.
  • Meet regularly with supervisor and attend department meetings.
  • Observe and report need to repair equipment, furniture, building and fixtures.
  • Use chemicals safely and appropriately, observing appropriate safety precautions, at all times.

Job Requirements/Qualifications:

  • High school diploma or equivalent preferred but not required
  • Previous experience working with elderly people, preferred but not required
  • Strong interpersonal communication skills required
  • Dependable, cooperative, and highly motivated to work with and care for the elderly
  • Sign up and start CNA classes within 60 days of being hired/transferred into this position

Job Description:

Housekeeping/Laundry Aides will perform day-to-day activities of Housekeeping and Laundry Department to assure that facility is maintained in a clean, safe, and comfortable manner, and that an adequate supple of linen is on hand at all times to meet the needs of the residents.

Duties Include:

  • Follow work/cleaning schedules.
  • Perform day-to-day housekeeping functions as assigned.
  • Perform specific tasks in accordance with daily work assignments.
  • Report all accidents, incidents, defects, or other hazards to supervisor immediately.
  • Attend and participate in meetings and staff development as applicable or as directed.
  • Follow establish policies and procedures to ensure safety and sanitation.
  • Keep supervisor informed of supply needs, including those of the area you work in.
  • Assist others in lifting heavy equipment, supplies, etc. as directed or requested.
  • Maintain cleanliness of work/supply carts, equipment, etc. Store appropriately at end of shift.
  • Clean furnishing, fixtures, ledges, mirrors, windows, heating and cooling units, etc. in resident rooms, recreational areas, etc. weekly as instructed.
  • Clean, wash sanitize, and polish bathroom fixtures; ensure that watermarks are removed.
  • Clean floors (sweep, dust, wet mop, disinfect, vacuum carpet), walls and ceilings.
  • Clean hallways, stairs, elevators, and handrails.
  • Dispose of trash properly; reline trash receptacle with plastic liner.
  • Clean vacant rooms as assigned.
  • Perform isolation cleaning procedures in accordance with established infection control procedures. 
  • Discard infectious wastes into appropriate containers.

Job Requirements/Qualifications:

  • High school diploma or equivalent preferred.
  • Experience as housekeeper in long-term care or hospital setting preferred
  • Knowledge of cleaning appliances, tools, and procedures.
  • Ability to work independently, with others, and to follow instructions.
  • Special interest and desire to serve the aging population.
  • Ability to read, write, speak, and understand common communication methods.
  • Requires the ability to lift up to 60 pounds occasionally and up to 35 pounds frequently.
  • Frequent walking, sitting, talking, hearing, and repetitive motions required; occasional reaching, kneeling standing, pulling, lifting, grasping, feeling required
  • Adherence to universal precaution is required
