Greencroft Goshen’s health facilities provide state–of–the–art health care techniques and technology in an inviting and supportive residential environment. Our setting is appropriate for individuals who are recovering from surgery or illness, need rehabilitation, or require ongoing long-term nursing care. Our restorative care philosophy reinforces the healing and rehabilitation process and promotes independence. A full range of activities, chaplaincy, and social services are provided.
The Thelma A. Schrock Homestead – Adult Day Services
Adult Day Services at the Greencroft Goshen campus provide care for up to 35 individuals from Elkhart and Kosciusko counties. You can choose from flexible part time or full time attendance, transportation, meals, financial assistance (for those qualifying), and volunteer opportunities. This "day family" experience aims to increase mental function, maintain independence, and enhance the self-esteem of the participant, and provide a break for the caregiver.
At The Meadows health care center we offer short- and long-term skilled nursing care as well as comprehensive rehabilitation programs. Rehab services include physical, speech, and occupational therapy. Because The Meadows is located within the Southfield Village campus community, residents can remain among friends in familiar surroundings should a higher level of care be needed.
The Meadows employs a highly skilled staff whose goal is to provide excellent care. Comprehensive rehabilitation services are offered, including physical, occupational and speech therapy.
Private and semi–private rooms are available along with these rehabilitation services. For additional convenience, The Meadows at Southfield Village is also Medicare/Medicaid Certified.
Our comprehensive approach to rehabilitation means reshaping lives and restoring lost abilities after an illness or injury. It means treating the whole patient to return people to their optimum level of function and open the door to a better quality of life. Services include physical, occupational, and speech therapies available seven days a week; an interdisciplinary treatment approach; and family involvement in the treatment and care planning processes.
With an emphasis on rehabilitation, our staff provides innovative treatment approaches to assist residents in returning to their normal and viable lifestyle. Our rehabilitation professionals include occupational therapists, certified occupational therapy assistants, physical therapists, physical therapy assistants, and speech language pathologists, all of whom are licensed, working as a team to provide comprehensive therapy services in a caring, home-like environment.
The skilled nursing and rehabilitation services at Oak Grove offer comprehensive physical, occupational, speech, and respiratory therapies provided by licensed therapists for short- and longer-term care.
