Our History

1995

Board of Directors appointed.

1996

Through the vision and leadership of Wilbert Hamstra over $3.25 million raised through local private donations to establish the community.

1997

Groundbreaking began in 1997 with construction finishing in 1998.

1999

Oak Grove began operations in DeMotte, Indiana. Services include Assisted Living, Alzheimer’s care, skilled care and long-term care.

2007

January 1, affiliated with Greencroft Communities.

2013

July, Oak Leaf Rehabilitation Center 11-room addition opens.

2017

A $4 million capital campaign kicked off to raise funds for a dedicated memory care unit, to expand assisted living and renovate existing facility.

2018

Groundbreaking began on the Memory Care expansion project.

2021

Memory care unit, Oak Street expansion and opening.
